LIBERAL, Kansas — Southwest Medical Center is reminding patients that Liberal Family Medicine Care Center has a new phone number for appointments, prescription refills, and all patient inquiries.

Patients can now reach the clinic by calling (620) 556-4550.

Family nurse practitioners Heidi Riley, APRN, FNP-C, and Tiffany Landa, APRN, FNP-C, have been caring for patients at Liberal Family Medicine Care Center since relocating their practice earlier this year to 121 E. 11th Street in Liberal. The clinic continues to provide comprehensive primary care services and is accepting both existing and new patients.

“We want to ensure patients have the correct contact information for the clinic,” said Amber Williams, CEO of Southwest Medical Center. “Heidi and Tiffany have successfully transitioned to Liberal Family Medicine Care Center, and we encourage patients to update their records with our new phone number.”

Liberal Family Medicine Care Center accepts most major insurance plans, including Blue Cross Blue Shield, Medicare, Kansas Medicaid, and Oklahoma Medicaid.

Clinic hours are:
• Monday through Thursday: 8:00 a.m. to 5:00 p.m.
• Friday: 8:00 a.m. to 12:00 p.m.

Riley brings more than 22 years of experience as a family nurse practitioner serving the Liberal community, while Landa has provided family healthcare throughout Southwest Kansas for 14 years. Together, they continue their commitment to delivering compassionate, patient-centered care for individuals and families of all ages.
