Public Health Notice From the Seward County Health Department

The Seward County Health Department would like to make residents aware of the latest information regarding the H1N1 Influenza Vaccines.

Due to limited vaccine supply, the Health Department is only vaccinating those who the CDC and KDHE categorize as high risk patients.

These patients include

?Ł Caregivers and siblings of children under 6 months of age

?Ł Children 6 months to 4 years of age

Beginning Wednesday, November 4, 2009, anyone who meets the above risk categories are welcome to come to the Seward County Health Department to receive the H1N1 Influenza Vaccine free of charge.

Hours of the clinic: Monday to Thursday 8:30am to 4:00pm, Fridays 9:00am to 11:00am

The form needed for the vaccine may be downloaded from the Kansas Department of Health and Environment website, for English http://www.kdheks.gov/flu/download/KS_H1N1_consent_form_template.pdf.

For Spanish http://www.kdheks.gov/flu/download/KS_H1N1_consent_form_template_Spanish.pdf.

Please print this form and bring it with you to speed up the process at the Seward County Health Department. This form will be available at the clinic for those that are unable to download it.

Children, 6 months to 18 years of age with underlying health conditions, need to contact their Pediatrician for the H1N1 vaccine.
