Southwest Medical Center’s 15th Street Family Care Center is offering Sports Physicals for all school activities for only $25. The Center is located at 555 W. 15th Street and the physicals will be given Wednesday, July 1th, and Thursday, July 17th from 1:00 pm – 5:00 pm, no appointment needed. Cash, debit, or credit cards will be accepted. Private insurance and Medicaid accepted. Physical forms must be completed and signed by a parent or guardian before the physical can be done.
